Accepted Insurance
The Center for Medical Dermatology and Immunology Research currently accepts insurance from three primary insurance companies. Please bring proof of your insurance to all appointments. If your insurance requires a referral, obtain an authorization form from your primary care physician. Additionally, we will need a copy of your driver's license or other valid identification.
If you choose to be seen without insurance or with an insurance plan we do not accept, you will be responsible for the cost of the visit and any procedures on the day of your appointment.
